Due to the international regulatory restrictions, MBC Financial Services Ltd (“MBCFX”) Trading Agreement cannot be processed unless all applicable sections below are complete.

MBC FINANCIAL SERVICES LTD’s Anti-Money Laundering Policy To comply with MBC FINANCIAL SERVICES LTD’S AML policy, government photo identification with current address (e.g. driver’s license, passport, any Government issued valid ID, or national identity card), utility bill or other such documentation as may be appropriate to ascertain and verify a person’s identity must be submitted with the account application.

Required Forms and Signatures

1 . Account Application. Please carefully complete, provide all supplemental information and sign the Account Application form on page 10 of
MBCFX New clients account application

2 . Photo ID Please provide a current copy of the primary contact’s government issued legal photo ID. Acceptable forms of photo ID, the acceptable forms of photo ID will be limited to ONLY current passports and government issued identifications.

3 . Proof of Address: Electricity Bill, Bank statement, and Lease contract

4. Discretionary Trading Authority/Power of Attorney. If your account will be guided by a third party, then please read and complete the Discretionary Trading Authority/Power of Attorney form located on page 13 of this packet.
